Office of the Director, Institute of Medical Sciences, BHU Varanasi-221005
sending a self addressed unstamped envelope (24 x 30 cm) and a DD of
Rs.100.00 in favour of “Director, IMS, BHU” payable at Varanasi
by post
Envelope for the form-request should be marked as “MD/MS FORM-REQUEST”.
